We are seeking a resourceful Patient Navigator who can help patients understand and complete the next steps in their care.
What You'll Do
- Help patients coordinate referrals, testing, follow-up, and outside records.
- Explain administrative next steps in clear, patient-friendly language.
- Identify barriers and connect patients with appropriate practice or community resources.
- Track open items and communicate updates to the care team.
- Document interactions accurately and protect patient privacy.
What We're Looking For
- Healthcare coordination, community health, or medical office experience preferred.
- Empathetic communication and practical problem-solving.
- Strong organization and follow-through.
- Comfort communicating by phone and electronic systems.
- Bilingual English/Spanish strongly preferred.
